This tag provides responders with a durable documentation tool for recording patient condition, treatment details, contamination status, transport information, and destination details during MCI operations.

The wristband-style configuration helps maintain patient identification throughout the incident while the attached triage tag supports continued assessment, care documentation, and patient movement tracking.

Wristband Triage Tag Configuration

This version includes an integrated patient wristband that can be applied to the patient while the triage tag remains available for documentation. The wristband helps preserve patient identification as the patient moves between treatment areas, transport units, and receiving facilities.
